When attorneys review a medical record, they are typically looking at the clinical story presented in the chart.

But sometimes there is another story behind that record.

An Electronic Health Record (EHR) audit trail can provide information about activity occurring within the electronic record that may not be apparent from the medical chart alone.

  • When documentation was entered
  • When an entry was modified or amended
  • Which user accessed or interacted with portions of the record
  • The sequence and timing of certain EHR activity
  • Documentation activity occurring before or after significant clinical events

Why does that matter?

Imagine reviewing a case involving a deterioration in a patientโ€™s condition.

The medical record may tell you what was documented.

When was it documented?
Was it entered contemporaneously or later?
Was something subsequently modified?
Who interacted with the record, and when?

Those distinctions can become important when reconstructing the clinical timeline.

But an audit trail should not be interpreted in isolation.

The real value comes from correlating the electronic activity with the clinical record, timeline, orders, medications, communications, and other available evidence.

That is where clinical interpretation matters.

As a Legal Nurse Consultant, I see audit-trail analysis as another layer of medical-record review โ€” one that can help attorneys understand not only what the chart says, but also the electronic history surrounding how that record developed.
